Summary: Under the direct supervision of the Director of Behavioral Health, the FEP Case Manager/Outreach Specialist provides the initial support for those individuals who may be experiencing their initial episode of psychosis. Case management duties include case finding, case management, assessment and planning, and case coordination between client, family, and community resources. Works to develop and facilitate rehabilitation activities, including skills development and recreational opportunities. Due to the small caseload of the individuals identified for FEP a large majority of the time will be spent performing community outreach to develop referral sources for the program. The position will perform outreach in high schools, colleges, religious organizations, community groups, and other identified referral sources. Must be comfortable and be able to develop relationships with a wide variety of individuals.

- This position requires compliance with MSHS' compliance standards, including the Code of Conduct, Compliance Program, policies and procedures, and departmental requirements. Such compliance will be considered part of the regular performance evaluation.
- Maintains regular contact with outside resources and providers. Advocates on behalf of the consumer, assist qualifying clients/families in identifying, obtaining, and securing needed employment, entitlements, supports, and services.
- Maintains accurate client service records and distributes completed information and medical records to service providers within the established time frame.
- Review treatment plans, monitor progress, and provide necessary follow-ups as required by the current Division of Behavioral Health regulations.
- Provides and participates in interdisciplinary quality assurance, utilization review, treatment planning, treatment assessments, and assessment of function.
- Monitors and coordinates client services and the service delivery system to ensure the care is appropriate, medically necessary, clinically justifiable, and active treatment.
- Provides direct services, including individual and group skills development and crisis intervention. Researches and develops curriculum and prepares class materials for skills-building classes.
- Participates in the FEP Team and FEP Team meetings.
- Performs planned outreach activities with a monthly schedule of places to introduce the program and provide educational materials and other giveaway items.
- Obtains referrals from a minimum of 3 different sources throughout the year.
- May provide training as needed.
- Remains familiar with and adheres to published Agency policies.
- Provides access to community resources for clients as scheduled utilizing Agency vehicles.
- Provides education about mental health and services to outside agencies and organizations as requested.
- Actively works to ensure implementation of the values and principles of psychosocial rehabilitation.
- Other position/program-related duties as assigned.
- Demonstrated ability to communicate effectively, both orally and in writing.
- Comfortable doing presentations and talking with a large number of people.
- Ability to independently plan, schedule, and conduct program activities.
- Knowledge of standard office equipment, including personal computer and standard word-processing programs experience.
- Work is performed primarily inside a heated, ventilated, and well-lighted environment.
- Job is considered low-risk for hazard exposure.
- MSHS is a nonsmoking campus.
- Requires sitting, standing, bending, reaching, lifting under 50 lbs., speaking and driving.
- Requires manual dexterity to operate standard office and other equipment.
- Requires normal hearing range, ability to speak, and vision acuity.
- BA/BS degree preferred in Social Work, Psychology, Human Services, or related field with some work experience. 5 years of relevant work experience may be substituted for education.
- Education & relevant work experience equivalent to 5 years may be considered.
- Clear background check (subject to DHSS background check)
- Current tuberculosis screening test result
- Current driver's license; clean driving record history
- BLS certification must be obtained within two weeks of hire. CPI (Nonviolent Crisis Intervention) certification must be obtained when offered through the agency; it may substitute the current MANDT certificate for CPI.
