Hospitalist - Inpatient Care Coordinator

Summary

Williamson Health is seeking a Hospitalist - Inpatient Care Coordinator to provide high-quality, cost-effective, and evidence-based medical care to hospitalized patients. This role involves coordinating care for patients at discharge and facilitating hand-off to the outpatient physician. The ideal candidate will demonstrate strong leadership, managerial ability, and good interpersonal relationships.

Responsibilities:

1. Assess, evaluate, and clinically manage patients
2. Provide active management of patient's condition through diagnostic, preventative, and therapeutic medical services
3. Collaborate with healthcare professionals to determine the best care plan for the hospitalized patient
4. Facilitate timely admission, discharge, and transfer of patients
5. Provide information, education, and support to patients and their families
6. Prescribe or recommend pharmacologic and non-pharmacologic therapies
7. Communicate with primary care physicians or specialists as needed
8. Participate in achieving patient safety goals, quality measures, and clinical outcomes consistent with organizational objectives
9. Maintain compliance with all applicable regulations such as Joint Commission and CMS Conditions of Participation
