Director Case Management

HCT seeks an experienced nursing professional to serve as the Director of Case Management for a 350+ bed hospital in California. Responsibilities include, but are not limited to:

• Oversight of the daily operations of the Care Management and Social Services departments.
• Developing and ensuring adherence to departmental best practices and policies as well as providing education.  
• Establishing outstanding rapport and coordinating with physicians, clinicians, health plans, and local providers such as skilled nursing facilities, long-term care facilities, home health agencies, and ambulance companies.

Requirements:

• Education: BSN degree required; Master's degree in related field preferred; RN required.
• Experience: Minimum 5 years’ progressive experience in a Management or Administrative capacity in Hospital Case Management leadership.  Knowledge of DRGs, Medicare Regulations, Medical Necessity Criteria, and associated knowledge specific to Case Management. Experience with State & Federal Regulations, including Title 22 and other regulatory requirements. 
• Licensure: Current CA RN license required.
• Certifications: BLS required. Specialty certification(s) in the areas of Case Management and/or Administration is preferred.
• Computer Skills: Meditech.
