Care Coordinator (Full Time)
The Care Coordinator is a member of a multi-disciplinary Certified Community Behavioral Health (CCBHC) Team. The CCBHC Team supports clients of Community Care Alliance (CCA) and their teams with necessary care coordination and access to resources as well as triaging clients who are dealing with complex medical and behavioral health and/or substance issues and require a more intensive outpatient, home and/or community-based intervention. A CCBHC team coordinates care across the spectrum of health services, including access to high- quality physical health (both acute and chronic) and behavioral health care, as well as social services, housing, educational systems, and employment opportunities as necessary to facilitate wellness and recovery of the whole person.
Care Coordinators play a critical role on the team by providing health care coordination, community resources and referrals. The Care Coordinators will network and serve as liaison with internal and external community resources concerning social determinants of Health in order to improve and enhance CCBHC integration of care. Examples include but are not limited to: (External) Thundermist, Landmark Medical Center, PCP offices, Managed Care organizations (Internal); Safe Haven, Family Support Center, Social Determinants of Health meetings and following up on aftercare referrals for continuity of care. The Care Coordinators can see clients onsite, in their homes and/or in the communities in which they live, while providing culturally sensitive approaches to help clients achieve their behavioral health and rehabilitation goals. The Care Coordinator also possess the skillset to assist in the needs with staff development.
The Care Coordinator must possess a driver's license, an approved driver's record, and reliable transportation required.
EDUCATION AND TRAINING REQUIRED:

  • BA/BSW in a human service-related field preferred.
  • Knowledge of substance use and mental health is preferred.
  • Experience with SPMI population preferred.
  • Knowledge of community resources in northern RI preferred.
  • Experience with Electronic Health Record systems is preferred.
  • Bilingual candidates are encouraged to apply.

  • Competently and consistently completes work accurately and thoroughly.
  • Ability to be a self-starter, work with minimal supervision and continually seek new/better methods to organize/complete work.
  • Ability to transport clients and perform home visits.
  • Able to perform in a fast-paced environment and meet or exceed expected service levels including health home targets.
  • Effectively accomplishes duties by working with others, including team members, other CCA staff and/or outside agencies.
  • Regularly available for work, reliable and on time for all things scheduled.
