The Breast Cancer Screening Patient Navigator is responsible for the care coordination of designated BMS patients who are eligible for mammograms and have not yet completed the screening. Acts as an advocate, educator, and support person to remove barriers to screening completion. Pulls patient lists from the population health tool (i2i) monthly to identify patients who should receive a Mammogram to determine follow-up and address barriers to screening. Provides navigation for diagnostic exams placed in the EMR, schedules patients for mammograms with Radiology centers, and assesses and addresses any barriers to completion. Manages the BCS clinic days with Radiology centers, sends monthly text messages using the opportunity lists, screens patients on the opportunity list and provides resources for barriers while identifying and connecting to external resources. Always demonstrates in-depth knowledge and appropriate resources to assist patients. Maintains educational tools to inform patients about breast cancer risk, the importance of genetic counseling, and identifying breast cancer signs and symptoms.

  • AA Degree
Experience:
  • 2 years of recent primary care experience in an inpatient or ambulatory care setting
