Position Title: Licensed Practical Nurse (LPN) - (Part-time, Traveling)
Department: Partners in Parenting (PIP)
Reports to: Program Director
Position Objective and Summary :
The Licensed Practical Nurse addresses the medical care coordination needs of youth within PIP program and community settings.
Essential Functions and Responsibilities :
  • Trains staff and youth on various health care topics.
  • Keeps accurate and complete case notes on youth.
  • Assist team with the completion of Asthma Action Plans.
  • Assist with the development of service plan goals for youth and children with high-risk/chronic health care needs.
  • Educate youth on the importance of attending routine prenatal care and the effects of cigarettes/drugs/alcohol on the unborn fetus.
  • Educate youth on all family planning options highlighting the pros and cons of each.
  • Review all prescriptions and medication dosages for youth on psychotropic meds. Provide education to the team regarding potential side effects and coordinate with placement provider if needed.
  • Coordinate psychiatric and psychological appointments.
  • Assist youth with medication refills (ex, high blood pressure, eczema, diabetes, etc.)
  • Administer hearing and vision screenings for youth and their children.
  • Plans and implements health management protocols for youth with chronic health problems.
  • Liaison with community hospitals to coordinate care for youth and young adults.
  • Promotes and assists in the control of communicable diseases through preventive immunization programs, early detection, surveillance and reporting of contagious diseases.

  • Performs other related tasks as required or assigned.
Education/Job Experience/Certification :
  • State of Illinois LPN licensure.
  • Current CPR certification.
  • Two (2) years' experience in related work.
Special Knowledge and Qualifications :
  • Proven ability to work effectively with young people.
  • Understands trauma-informed practice.
  • Effective verbal and written communication skills.
  • Ability to work both independently and a part of a high functioning team.
  • Experience and ability to use computers for data entry, communication and report writing.
  • Car and valid Illinois driver's license and insurance required.

Work Environment : Administrative offices and community.
