Elwyn Adult Behavioral Health Services is seeking a compassionate and dedicated Case Manager to join our dynamic Behavioral Health Drop-In Center in Raleigh, NC. You'll provide critical support, coordination, and resources to help individuals with serious mental illness and co-occurring disorders achieve their recovery goals and live more independently.
$40,000 - $41,000 per year, comprehensive benefits
Monday - Friday, 8 am - 4 pm
The Drop-In Center:
- Operates under a blended model of a traditional clubhouse and peer-run recovery center, offering a cost-reimbursed service.
- Provides a comprehensive program that goes beyond mental health/addiction recovery counseling.
- Offers daily programs including:
- Physical health education and support
- Leisure activities
- Structured skills training groups
- Peer recovery services
- Access to Elwyn's creative arts program, Studio 35
- Creates a space of hope and opportunity for members with mental illness to achieve their full potential.
What you'll do:
- Advocating for Clients : You'll be a champion for your clients, ensuring their voices are heard and their needs are met. Working collaboratively with the Drop-In Center Team, you'll develop individualized plans to facilitate recovery and empower clients to reach their full potential.
- Building Strong Relationships : You'll develop strong therapeutic relationships with clients, creating a safe and supportive space where they feel comfortable discussing challenges and setting goals. Your ability to connect and engage effectively will be crucial in their journey towards recovery.
- Utilizing Evidence-Based Practices : You'll employ evidence-based interventions, including the PRISM model and Cognitive Behavioral Therapy (CBT), to deliver effective support to your clients. By staying current with best practices, you'll ensure clients receive the most up-to-date and effective treatment approaches.
- Providing In-Vivo Care : You'll be directly involved in supporting clients in their daily lives (in-vivo care). This includes tasks like medication monitoring and education, providing psychoeducational activities, and helping clients develop skills for daily living (ADL/IADL), community integration, social interaction, and employment.
What we're looking for:
- Bachelor's degree in a Human Services field , such as Social Work, Psychology, Rehabilitative Counseling, or a closely related discipline (Public Health, Nursing).
- At least two years of experience in human services, preferably in case management.
- Strong understanding of recovery-oriented and person-centered care principles.
- The ability to establish and maintain effective therapeutic relationships and creatively engage clients in discussions and planning around recovery and personalized goals.
- A thorough understanding of the clinical symptoms and behavioral aspects of SPMI.
- Experience with crisis intervention principles and practices to effectively de-escalate situations.
- Knowledge of prescribed medications used in the program, their effects, and the ability to monitor client self-medication.
- Ability to manage a caseload effectively and prioritize tasks in a fast-paced environment.
- Proficient in documentation and record-keeping, with experience in electronic health record (EHR) systems a plus.
- A valid driver's license in good standing .
