Lead the Opening of our Brand New Senior Primary Care Office in Wesley Chapel, Florida.

Conviva Senior Care Centers , a subsidiary of Humana Inc., is seeking a Family, Internal, or Geriatric Medicine Physician for our newest office. Grow with us as we expand our reach to help more Seniors live better, healthier lives.

You have the opportunity to take a hands-on approach, addressing patient needs inside the exam room and looking beyond to understand how social determinants of health are impacting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a multidisciplinary approach.

A day in the life of a Conviva physician:

  1. Monday-Friday 8:00am-5:00pm; no weekends, and 8 paid holidays off
  2. Flexible schedule available, if needed
  3. Join an experienced team with two Advanced Practitioners and a Care Team
  4. See 12-15 patients per day . Opportunity to spend more time with patients
  5. Longer authorized appointments (45 minutes)
  6. Acts as a lead and active participant and key source of medical expertise with the Care Team through daily huddles
  7. Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care

Required Qualifications:
  1. Doctor of Medicine or Osteopathic Medicine
  2. Board Certified/Board Eligible in Family, Internal, or Geriatric Medicine
  3. Valid, unrestricted license in the state of desired employment (or willing to obtain)
  4. Current, unrestricted DEA license

Preferred Qualifications:
  1. Two to five years directly applicable experience preferred
  2. Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
  3. Knowledge of Medicare guidelines and coverage.
  4. Knowledge of HEDIS quality indicators.
