MDS Coordinator Responsibilities:

The MDS Coordinator plays a critical role in ensuring accurate resident assessments and optimizing care plans. Key responsibilities include:

  • Conducting and coordinating the development and completion of resident assessments (MDS) in accordance with current regulations and guidelines.
  • Ensuring timely and precise completion of the MDS process.
  • Collaborating with the interdisciplinary team to create personalized care plans for residents.
  • Monitoring and documenting resident progress and responses to care plans.
  • Actively participating in the Quality Assurance and Performance Improvement (QAPI) process.
  • Communicating effectively with residents, families, and healthcare providers to ensure seamless care coordination.
  • Staying current on MDS regulations and guidelines to maintain compliance.

Pay range: Up to $84,000 annually

Requirements:

  • Current RN (Registered Nurse) license
  • RAC-CT (Resident Assessment Coordinator-Certified) certification is required
  • Strong understanding of MDS 3.0, RAI process, and care planning
  • Excellent organizational, communication, and interpersonal skills
  • Ability to work independently and collaboratively within a team
  • Commitment to providing high-quality care to residents
