Community Health Worker

Upper Great Lakes Family Health is seeking a Community Health Worker to serve as a liaison between our patients and health & social services within the community.*Responsibilities, included but not limited to: *Identifies and tracks patients with barriers to care i.e., transportation, financial, housing/shelter needs, etc.Identifies and tracks patients with health literacy issues and ensures the patient is provided with the appropriate resources.Provides patient education and connects patients to community resources.Partners with Care Managers to provide targeted interventions to avoid hospitalization and emergency department visits.This position is a .6 FTE (scheduled 48 hours per pay period) located within our Pediatrics clinic.
