Serves as a non-clinical resource and liaison for patients and their families. Assists with navigating all aspects of the healthcare system. Uses knowledge of the unit or clinic to resolve issues and identify ways to increase patient satisfaction. Collaborates with department managers to meet patient expectations, achieve quality outcomes and build a patient focused culture.

Assists with quality improvement efforts to improve care for gender diverse individuals at the Family Health Center in San Francisco by conducting and reviewing surveys and focus groups with current patients. Assists in recruiting and communication with members of a Patient Advisory Committee. Over time, will provide non-clinical support for our gender diverse patient population, including overseeing panel management to ensure care gaps are being met, helping patients navigate our complex healthcare system and community resources, and serving as a liaison with providers. The Gender Affirming Care Coordinator (Care Coordinator) will be an essential part of the clinical care curriculum based at the residency training clinic, the Family Health Center at the Zuckerberg San Francisco General Hospital and Trauma Center, funded by a new Office of the Director, NIH (OD) National Library of Medicine R25: Queering the Curriculum: Preparing Family Medicine and Other Primary Care Residents to Care for Transgender, Non-Binary, and Gender Diverse People.

In this role, the Care Coordinator supports the creation of a comprehensive longitudinal curriculum for family medicine residents focused on primary and specialized care for patients across the gender spectrum, including the following responsibilities:

  • Conducts surveys and focus groups with transgender/gender diverse (TDGD) patients in order to recommend changes in clinic structure and services to better serve TGD patients, both adults and children.
  • Serves as a non-clinical resource and liaison for TGD patients (and their families when applicable).
  • Assists with navigating all aspects of the healthcare system, including connecting with community resources.
  • Liaisons with Gender Health SF to facilitate improved coordination of care.
  • Uses knowledge of the unit or clinic to resolve issues and identify ways to increase patient satisfaction.
  • Develops library of resources, and orientation information for patients.
  • Opportunity to help disseminate best practices for teaching clinics serving gender diverse patients.

The Care Coordinator will help ensure continuity for our gender diverse patient population, including implementation of best practices for panel management to ensure care gaps are being met, helping patients navigate our complex healthcare system and community resources, and serving as a liaison with resident providers. In this role, they will support resident education on the healthcare needs of gender diverse individuals.

The Care Coordinator will help with writing up best practices for teaching clinics serving gender diverse patients that will be part of the curriculum package for dissemination. The Care Coordinator will assist with quality improvement (QI) efforts to improve care for gender diverse individuals by reviewing patient surveys and conducting focus groups with current patients. The Care Coordinator will also assist in recruitment, communications, and coordination of the Patient Advisory Committee members.

Other duties as assigned. Hours may include four (4) likely evening commitments a year for Patient Advisory Committee meetings.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $55,200 - $95,900 (Annual Rate).
