Position Summary:
Under the supervision of SH Coordinator, responsible for coordinating an array of services and provide case management for clients within supportive housing. Conduct person-centered planning, linking and coordination of services, facilitation, monitoring, assisting with court involved cases and hearings and advocacy responsibilities regarding the needs of individuals who chose case management services. Participate in cultural and community events and activities on a regular basis. Must adhere to mandated reporting laws. The Supportive Housing Program operates using both Housing First philosophy and Harm Reduction approaches. The residents of supportive housing are living with substance use disorders, mental illness or psychological disability. Employees must be guided by the principles of trauma informed care: physical and emotional safety, trust, choice, collaboration and empowerment. This position is funded under the Indian Housing Block Grant which is a regular recurring formula grant dispersed through the U.S. Department of Housing and Urban Development.
Essential Duties and Responsibilities:
  • Conduct needs assessments; coordinate, monitor, evaluate and advocate for appropriate services for clients.
  • Provide culturally appropriate clinical services to individuals, families and groups with various co-occurring disorders and age groups.
  • Assist clients in addressing health issues that may impact individual's achievement of life goals and becoming contributing member of community.
  • Develop, arrange, monitor, and evaluate client service plans to include measurable and date-specific goals stated in client's service plan. Strong written and communication skill is required.
  • Perform crisis intervention to resolve emergency problems of clients.
  • Complete documentation in accordance with policies of case management, recommendations, and services provided.
  • Assist clients with budget planning, maintain housing, SSI applications, employment and other identified needs pertaining to case management.
  • Facilitate support group(s) based on tenant needs and interests.
  • Lead therapeutic and recreational activities with tenants and their household members.
  • Participate in mandatory staff meetings and trainings as required.
  • Initiate and coordinate with other agencies and provide consultation to staff working with clients.
  • Establish linkage agreements with outside community agencies.
  • Participate and arrange Care Coordination meetings to address the needs of clients.
  • Work cooperatively with professionals and paraprofessionals while utilizing local and tribal resources.
  • Advocate on client's behalf to other community providers.
  • Provide individual, family, and/or group care and supports.
  • Establish positive and effective relationships with members of outside community agencies and programs.
  • Adhere to client confidentiality and comply with department policies and federal regulations under the Privacy rules of the Health Insurance Portability and Accountability Act (HIPAA).
  • Provide excellent customer service for all internal and external customers of the operations at all times.
  • Must maintain strict confidentiality and present a positive, professional demeanor and image at all times.
  • Be creative and innovative with solutions focused on Tenant retention.

Minimum Qualifications:
  • Bachelor's Degree in Social Work, Counseling, or Human Services related field.
  • One year of program related experience.
  • An equivalent combination of education and directly related experience may be considered.
  • Experience in working collaboratively across multiple organizations and community systems.
  • Excellent communication skills.
  • Excellent computer and office machines skills.
  • Must be willing to work a flexible/adaptable schedule as meets the needs of the program.
  • Must be able to pass background check to meet and maintain the employment eligibility requirements as they pertain to the position.
Desired Qualifications:
  • Experience with substance abuse treatment protocols, the judicial/legal system, and co-occurring experience with adults, youth, and families preferred.
  • Experience with Native American traditions and culture preferred.
License, Certification, or Special Requirements:
  • Obtain HIPAA certification within five business days from date of hire.
  • Must have a valid Michigan Driver's License and must be able to obtain and maintain a Tribal Driver's License and qualify for coverage by SCIT auto liability insurance carrier.
  • Native American preference shall apply to all positions.
Knowledge, Skills, and Abilities:
  • Knowledge of applicable federal, state, tribal laws, regulations and requirements.
  • Knowledge and experience with managing court involved cases (i.e., deferrals, guardianships, mental health certification, and petitioning).
  • Knowledge of the Indian Child Welfare Act, Child Protection Teams, and state and federal programs and their codes/laws.
  • Knowledge of alcohol and drug abuse and related psychological disorders and behaviors, including juvenile delinquency, ADHD, learning disorders, family dysfunctional and gang activity.
  • Knowledge of and experience regarding Native American mental health and substance abuse issues and be culturally sensitive.
  • Skill in interventions with individuals and groups.
  • Skill in effective listening and writing.
  • Skill in treating clients with courtesy and respect.
  • Ability to work under high stress situations.
  • Ability to investigate and analyze information and to draw conclusions.
  • Ability to react quickly and make effective decisions in an emergency situations.

Physical Demands:
  • Frequent use of hands, wrists, fingers associated with computer equipment.

  • Frequent walking and standing throughout the community to provide programming.
  • Normal visual acuity, ability to talk and hear.
  • Occasionally lift and/or move up to 20 pounds.
Work Environment:
  • Normal office environment, community and schools.
  • Mandated reporting of abuse and neglect can lead to personal challenges with exposure to victims of abuse.
  • High incidence of exposure to communicable diseases and blood borne pathogens evident in mental health and substance field.
  • Extended hours and irregular shifts may be required.
  • Travel may be required.
