Downtown Streets Team (DST) is looking for a passionate and outgoing Case Manager in Alameda to help us end homelessness through the dignity of work and the power of community. We run two proven programs in 16 cities that successfully transition the unhoused to self-sufficiency.

SUMMARY
The Case Manager will work directly with Team Members and Streets Team Enterprises Employees to navigate services and help them reduce the barriers to employment, housing, and all others related to self-sufficiency through strong organizational, problem-solving, and analytical skills, while managing priorities and workflow. The position will travel between Alameda County with a standing office in any of those cities depending on availability.

WHAT YOU’LL DO

  1. Travel between the following communities Alameda County will be constant part of the day to day operations
  2. Market Downtown Streets Team and Streets Team Enterprises to the community
  3. Build partnerships with local agencies to provide emergency, transitional, and permanent housing opportunities for Team Members and Streets Team Enterprises Employees
  4. Collaborate with businesses, community organizations, and volunteers to increase and improve employment prospects for Team Members and Streets Team Enterprises Employees
  5. Present new services and opportunities to entire DST Agency at designated Direct Services sections All Staff Meetings
  6. Track success metrics and input data into online systems such as HMIS
  7. Establish productive relationships with each and every Team Member and/or and Streets Team Enterprises Employees with no bias or prejudice towards race, religion, age, sex, disability, or sexual preference
  8. Uphold organizational values, work ethically and with integrity and follow company policies as laid out in handbooks
ADDITIONAL DUTIES AND RESPONSIBILITIES:
  1. Delegate your schedule to be able to support each community equally
  2. Effectively maintain established Case Management programs
  3. Create and deliver new Case Management programs to enhance Downtown Streets Team’s offerings
  4. Assist Team Members and Streets Team Enterprises Employees in the creation of job search collateral
  5. Find creative ways to remove barriers to self-sufficiency for Team Members and Streets Team Enterprises Employees
  6. Develop and support long-term career goals for Team Members and Streets Team Enterprises Employees, and track progress
  7. Help develop additional workshops to enhance empowerment and soft skills
  8. Help ensure that Team Members and Streets Team Enterprises Employees have long-term success by targeting a high retention rate (jobs held for one year)
  9. Represent the agency in the community and work with all partner organizations to achieve client goals
  10. Perform other duties as assigned by manager and assist with special projects as required
WHAT YOU'LL NEED:
  1. HS Diploma or equivalent
  2. Ability to perform job duties with minimal supervision
  3. Excellent listening comprehension, written and verbal communication skills
  4. Experience in Microsoft programs
  5. Using personal computer, keyboard, mouse and monitor
  6. Observation and problem solving; hearing, seeing, reasoning and decision making
  7. Basic mathematical ability
  8. Valid CDL, insurance and access to a reliable vehicle
  9. Regional travel 60% is required
  10. Physical requirements: Sitting, standing, walking, bending, stooping, pushing, pulling and lifting (up to 25 lbs.)
  11. Driving an insured and operational vehicle to designated communities (60% of job)
  12. Work environment and physical location may vary, depending on the space available and the needs of the community and the program.
  13. Employee can be based out of any of the communities they will be serving
