Description
I) Job Purpose
As a Help Me Thrive Navigator for the Transitioning Lives Collaborative (TLC) project, you will support youth and young adults ages 15-25 who are dually diagnosed with cognitive disability and a behavioral health diagnosis as they enter TLC services. You will assist with building a data base of resources and services youth and young adults can access to ensure they transition successfully to adulthood. You will support your TLC team mates by helping to address barriers youth and young adults may face in accessing services. You will impact youth, young adults, and our community by providing continuity of care and working toward wellness and recovery.
II) Essential Functions
A) Receives warm hand off from HMT phone navigators for referrals of youth and young adults who are dually diagnosed and potentially eligible for Transitioning Lives Collaborative.
B) Completes navigation surveys and screenings with youth and young adults to identify appropriate referrals both internally and externally. Refers youth and young adults that are eligible for TLC.
C) Admits youth and young adults in myAvatar. Schedules intake appointments for youth and young adults that are referred to TLC services through myAvatar scheduling calendar.
D) Collaborates and coordinates with TLC Family Interventionists and Family Advocate to ensure youth and young adults are receiving services to which they were referred. Assists with problem solving if a barrier to access occurs.
E) Enters documentation of all calls provided into myAvatar and/or FindConnect within established timelines and with accuracy.
F) Research and networks with referral sources in communities served by MHMR. Accesses CFS, MHMR, community, state and national resource data bases and online live binders for referral sources. Assists TLC Family Interventionists and Family Advocate in finding appropriate resources and services in the community to meet the needs of youth and young adults enrolled in TLC services.
G) Works closely with CFS Quality Management Resource Specialist to share and network referral sources. As requested, assists with updating of databases to include new service programs in the community for youth, young adults, and families. Maintains list of local programs and resources that have capacity to serve youth, young adults, and families.
H) Participates in regularly scheduled supervision and teaming meetings. Participates in internal and/or external committees, workgroups, networking, and referral source meetings as requested. Represents TLC at cross teaming meetings as requested and based on need.

III) Knowledge of Laws, Regulations, Policies/Procedures, Skills, and Abilities
A) Knowledge of typical, atypical, and disordered development, and medical diagnoses and conditions impacting children and youth prenatally through age 17, and transition age youth and young adults ages 15-25.
B) Knowledge of CFS program' eligibility requirements and services provided.
C) MHMRTC Policies and Procedures
D) FERPA and HIPPA compliance
E) Other state agency rules, regulations, and standards as appropriate (e.g., DFPS, HHSC, TCOOMMI, etc.)
F) Communicate effectively verbally and in writing.
G) Analytical and time-management skills or ability to be flexible and productive at a fast pace and produce quality work.
H) Knowledge of protective factors and family-strengthening strategies.
I) Ability to multitask, maintain focus, and complete assigned tasks within defined timelines.
J) Use of coaching and/or motivational interviewing and trauma-informed approaches while building capacity of children, youth, families, co-workers, and teams.
K) Knowledge of Texas laws regarding reporting adult and child abuse/neglect.
L) Working knowledge of community resources and case management principles.
M) Ability to proficiently operate a computer, software, and systems utilized by CFS, such as: Microsoft Outlook, Word, Excel & PowerPoint; myAvatar; UKG/Kronos; Teams; VPN; Salesforce, AgencyFinder and FINDConnect.
IV) Internal & External Customer Service
A) Interact and respond using a compassion mindset, awareness of need for felt safety, and consideration of all variables affecting families and teammates.
B) Work effectively with families/adults from various cultural, linguistic, socio-economic, cognitive, social-emotional, and educational statuses.
C) Build and maintain effective working relationships with internal and external customers, including families, characterized by mutual support, open communication, trust, and respect. Always considering what is best for families in decisions about treatment and support.
D) Discuss uncomfortable and potentially conflict-producing, stressful topics in a professional and effective manner with internal and external customers and maintain courteous interaction.
E) Must demonstrate compassion, trauma-informed care, and safety practices including suicide safe care
V) Travel
A) Adhere to MHMR Mileage and Travel reimbursement policy and any other aspect regarding travel.
B) This position requires daily travel including home, school, and community-based visits for families as assigned OR May require periodic travel within service delivery area to home, school and community-based visits or meetings. Refer to addendum for specific travel requirements.
C) Some out of town/state travel within the United States may be required for specified trainings, conferences, or meetings.

  1. Minimum Education: Bachelor's Degree
  2. Defined Education: Health and Human Services field of Applied Behavior Analysis, Child Development, Counseling, Early Childhood Education, Early Childhood Intervention, Education Psychology, Gerontology, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Registered Nurse, Rehabilitation & Rehab Studies, Social Work, Sociology, or Special Education.
  3. Preferences: Spanish Fluency
  4. Substitutions: Completion of college level coursework equivalent to requirements for QMHP credential
  5. Years' Experience: None
  6. Defined Experience: None
  7. License/Certifications: Qualified Mental Health Professional (QMHP) OR eligible for QMHP certification (based on minimum and defined education above)

  1. Agency Requirements
  1. All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.
  2. This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director.
  3. Assigned work hours may change as the needs of the agency change.
  4. The Functional Title of this position may change as the needs of the agency change.
  5. All work will be completed within the scheduled work hours. All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.
  6. MHMR reserves the right to change, add to or eliminate positions as it deems appropriate.

  8. Agency dress code is to be followed at all times.
  9. Physical on-site presence, including regular attendance and punctuality, is an essential function of this position. Any changes or adjustments to your assigned work schedule or shift hours must be approved by your supervisor in advance.
