**Job Description:**
- Function within the pharmacy department to assure effective, safe, and cost-efficient drug therapy.
- Prepare and distribute medications in compliance with Federal and State law.
- Provide patient counseling and education about medications and drug therapy outcomes.
- Offer consultation to medical and nursing staff regarding drug therapy.
- Maintain an appropriate drug inventory and participate in quality improvement activities.
- Evaluate prescriptions for appropriateness, legality, contraindications, interactions, and allergies.
- Provide clinical consultation and resolve prescription concerns with prescribers.
- Document clinical activities and interventions accurately.
- Verify prescription orders efficiently, ensuring accuracy and maintaining required documentation.
- Advise patients on their medications, ensuring understanding of indication, usage, and expected outcomes.
- Communicate with patients and healthcare providers to resolve issues and provide optimal care.
- Conduct chart reviews for prescription refills and medication management.
- Direct pharmacy support personnel and verify daily activities of pharmacy technicians.
- Serve as a role model and educator for pharmacy students and technicians.
- Participate in clinical and administrative activities, including quality control and improvement.
- Attend meetings and participate on committees as required.
- Ensure availability, preparation, and control of medications.
- Participate in health promotion and disease prevention activities related to drug use.
- Utilize knowledge of pharmaceutical sciences to review patient medical histories, labs, and medication orders.
- Remain knowledgeable about state and federal pharmacy laws.
- Communicate effectively with patients and staff, both orally and in writing.
- Demonstrate proficiency in computer application skills and math skills for accurate calculations.
- Work cooperatively with health system and pharmacy staff, exercising sound professional judgment.
- Handle frequent interruptions and adapt to changes in workload and schedules.
- Work independently, set priorities, make critical decisions, and respond quickly to emergencies.
