Job Description
Responsibilities:
- Provide HIV post-test counseling services to Bellevue patients who are diagnosed with HIV.
- Serve as a resource to Bellevue Medical Providers who are responsible for routinely offering and ordering HIV test to patients age 13 and older.
- Utilize the Bellevue HER and other electronic systems to receive patient referrals, to identify patients who engage in HIV risk behaviors for status neutral linkage and navigation services, and to assist patients in linking to HIV prevention services including PrEP in the Bellevue Sexual Health/Pride Clinic.
- Collaborate with administrative staff to ensure hat appointments, meetings, planned absences and other commitments are recorded.
- Document all patient contact, phone calls, voicemails, text messages, home/field outreach attempts, and face-to-face services promptly in the Epic HER.
- Document services and service outcomes as directed for data and grant reporting.
- Foster collaborative relationships with other professionals in and outside NYC Health + Hospitals
- Serve as role model of Bellevue's commitment to excellent patient care and customer service; embrace ICARE values and be proficient in the practice of ICARE customer service recovery behaviors.
- Participate in professional development and educational activities.
- Participate in hospital related activities as assigned.
- Perform other related duties.
1. A baccalaureate degree from an accredited college, including or suppIemented by twelve semester credits in health education or in health, social or biologcal sciences; or
2. A baccalaureate degree from an accredited college, and six months of full-time satisfactory experience in a health promotion or disease interventiodprevention program, performing one or more of the following: interviewing, conducting field investigations, assessing health risks, making referrals, or collecting and analyzing epidemiological data; or
3. A four-year high school diploma or its educational equivalent, and four years of full-time satisfactory experience as described in "2" above; or
4. Education and/or experience equivalent to " 1 ", "2'kr "3" above. Undergraduate college credit can be substituted for experience on the basis of 30 semester credits fiom an accredited college for one year of full-time experience. However, aII candidates must have a four-year high school diploma or its educational equivalent, and either twelve semester credits as described in "1" above or six months of experience as described in "2'' above.
5. One year of experience as a Public Health Adviser, Assignment Level I, or at least one additional year of experience as described in "2" above.
Additional Requirements:
1. For certain assignments, satisfactory completion of training in phlebotomy or other necessary procedures is required. Certification in HIV counseling may be required for certain other assignments.
2. A valid New York State Motor Vehicle Driver license may be required for certain assignments. If required, this license must be maintained for the duration of employment.
3. Public Health Advisers are required to have an annual health assessment.
