The Senior Community Health Nurse is in a highly responsible position with the general emphasis on school health and health education, using skills, knowledge, and ability to keep children at their maximum wellness. This position will be directly supervised by the Community Health Nursing Consultant.

The incumbent provides the following clinical health care services to students in the School Based Clinics/Clinical services: immunizations, family planning, STD/HIV counseling and screening, client assessment (interviewing and counseling), intervention and documentation; as well as drawing blood specimens and processing those specimens in the labs system. This position will enter data into a computerized charting system, monitor the system for compliance by Health Aides and LPNs; access logs, records, and forms from the system and accurately compile data for annual reports and the School Health Plan. The nurse provides referrals to specialty providers as necessary.

Monitors and manages the clinic, provides first aid, assists with prescription medications for students; takes vital signs and refers abnormal results to parents/nursing supervisor or medical personnel. Administers immunizations for students and adults. This position has permission to transport student’s identifiable information in a secure locked bag for work purposes only and must follow all HIPAA guidelines. This position may travel to multiple schools to provide services. This employee may act as designee in the absence of the Community Health Nurse Supervisor.

The nurse in this position will be responsible for the following screenings: BMI's (Body Mass Index), vision, hearing, and scoliosis. The nurse will refer students who perform unsatisfactorily on initial screening examinations for further evaluation and ensure that all screening dates are entered correctly in the computerized charting system and HMS. This position will follow-up on referrals made by teachers/staff for students with health problems. This employee uses the nursing process to solve problems and make referrals to appropriate agencies.

The nurse performs record reviews on all new students. This position develops care plans for students with chronic health conditions and for students taking medications.

The nurse provides health education/wellness programs that are approved by the school board and principal for students, faculty, and staff.

Attend teachers/principal conferences as needed to assist in the continuity of care of students. Act as health advisor and consultant in all health matters (IEP plan and 504), assist principals, teachers, counselors, and other agencies in family related health services. Provide oversight for school health liaisons at assigned schools.

Accurate completion of leave and attendance, Super Bill / HMS forms. 100% of EAR's shall be coded, with 97% completed within a 7-day time span. Assist in additional school activities, i.e.: health fairs, health promotion, etc. Assist with the completion of all school health reports for DOH. Perform all other related duties as directed by the supervisor.
