Submission Requirements
  • No candidates with an AZ home address on their driver's license will be considered - please include a copy in submission.
  • Minimum 24 months of current experience - REQUIRED.
  • Certifications must be American Heart Association OR client and active upon submission - MUST BE INCLUDED IN SUBMISSION - REQUIRED.
  • No Nursys Hits - REQUIRED.
  • Travel experience - Preferred.

  • Cover Page completely filled out.
  • Resume with all facilities, facility size, beds, trauma level, and units listed.
  • Skills Checklist specific to the discipline (must be less than a year old).
  • Active License (verification must be less than 30 days old).
  • Certifications (must be active at time of submission).
  • Driver's License.
Additional Details
  • Start Date should be on or very quickly after the Order Start Date listed.
  • All RTO should be listed on the cover page, must be less than 7 days.
  • RTO will not be approved in the first 2 weeks of assignment.
  • All facilities the traveler is open to working in should be listed on the submission.
  • All shifts the traveler is open to working on should be listed on the submission.
  • All departments the traveler is open to working must be checked on the cover page.
  • All candidates submitted should be preclosed and ready to accept.
  • Please let us know if the traveler withdraws or is no longer available.
