This position is responsible for promoting and coordinating activities to advance health equity and improve health outcomes for residents through developing partnerships, project planning, and implementation of projects that address social determinants of health (SDOH).Researches, compiles, analyzes, and shares data on health equity, health disparities, and SDOH with internal and external groups. This includes information related to health disparities within Hendry and Glades Counties, barriers to achieving health equity, best practices to address SDOH, and available funding opportunities.Promotes the development of programs that improve access to health care services. Facilitates a health equity coalition that includes organizations that serve the community. Engages partners in developing and updating health equity plans. Works with partners to coordinate and implement activities from health equity plans. Conducts presentations and provides updates at coalition meetings. Prepares agenda items and meeting minutes.Develops and submits program workplans, health equity plans, quarterly reports, and other documents as required, within established deadlines. Monitors and evaluates progress on health equity plan objectives.Coordinates technical assistance trainings and provides resources that provide better information dissemination, education about health disparities, improve cultural competency, and increase the quality of health outcomes and delivery of health care services.
