POSITION SUMMARY :

Under the direction of the Associate Director of Ambulatory Clinical Pharmacy, leads and supports ambulatory care specialists and functions in the development of enhanced ambulatory pharmacy services. Ensuring that the operational and clinical aspects of the respective specialty practice areas are in compliance with department policies and procedures, department mission and value statements, hospital core beliefs as well as any federal or state regulations pertaining to or governing pharmacy practice for the purpose of maximizing service and quality. In addition, ensures optimum patient outcomes related to medication use by preventing or resolving drug-related issues and optimizing the drug therapy of patients. Integrally involved in education of pharmacists, residents and students.

ESSENTIAL RESPONSIBILITIES / DUTIES:

Maximizes patient outcomes by optimizing both the operational and clinical aspects of care provided by ambulatory care specialists and reports regularly to the Associate Director of Ambulatory Clinical Pharmacy to ensure compliance with departmental quality improvement initiatives

Clinical Programming

  • Responsible for developing and expanding clinical pharmacy services in the ambulatory setting, collaborating with physician colleagues, and working with hospital and corporate leadership to obtain support for initiatives
  • Be instrumental to creative thinking and be a leader in the initiation of new clinical programs in their specialty
  • Develops and manages specialized projects with payers using an interdisciplinary team of providers in the development of health system-innovations to improve patient care and reduce cost
  • Contributes and participates in department and hospital committees
  • Demonstrates leadership role in the teaching programs of the department of pharmacy services related to ambulatory care
  • Facilitates outcomes research in the ambulatory care setting
  • Serves on the formulary committee, coordinating all pharmacy activities (MUE, guidelines, drug monograph etc..) of the committee and ensuring cost-effective drug utilization in collaboration with ambulatory care specialists
  • Assists the Associate Director of Ambulatory Clinical Pharmacy with the design, development, implementation and maintenance of new clinical quality improvement programs for the department and for the hospital’s accountable care organization, such as, but not limited to population health management and transitions of care.

Operational Management

  • Develops and implements workload statistics requiring management principals as well as integration and interpretation of data as a means of productivity enhancement
  • Oversees scheduling to maximize utilization of personnel and responds to deficiencies in patient care coverage to improve overall performance and patient outcomes, while meeting established service and program outcomes
  • Oversees the maintenance and utilization of the department information system for the purpose of maximizing function and application, constantly assessing breadth of function and guaranteeing that the system meets user needs
    • Participates in IT meetings as they relate to ambulatory practice areas and issues
    • Coordinates support for IT optimization tasks that will improve patient care and support patient safety
  • Identifies cost savings and revenue generating ideas through innovative methods within ambulatory care specialist group.
    • Optimizes billing services for clinical pharmacy services provided by ambulatory care specialists
    • Works to improve continuity of care between inpatient and outpatient setting
    • Helps improve prescription capture rate

Performance and Quality Improvement

  • Ensures and identifies quality assurance and improvement of all processes within purview
  • Ensures physician and other health care provider satisfaction with pharmacy services provided in clinic
  • Collects and maintains data on metrics that will be used to evaluate the impact of clinical pharmacy services provided in various areas and its direct impact on quality improvement and patient outcomes
  • Responsible for creating and maintaining a healthy work environment, where there is a culture of accountability, people feel supported, standards are applied and expectations are clear, and where leadership supports and leads, models behavioral standards and sets the example for how to do all things right.
  • Ensures clinical specialists have met minimum competency requirements for the role, and coordinates and supports continued education and training relevant to their specialized area
  • Assesses clinical specialist performance and prepares annual performance evaluations
  • Negotiates and strategize with the administrative director to intervene on practitioners who are non-compliant to practicing medication guidelines
  • Demonstrates ability to identify and resolve interpersonal or professional conflicts when dealing with members of the organization
  • Facilitates teamwork by coordinating regular team meetings within the ambulatory pharmacy group to
    • Support team-building and the sharing of best practices
    • Improve and maintain effective communication amongst the team
    • Advance projects to completion
    • Identify and implement new opportunities for advancing ambulatory clinical pharmacy practice

Professional Development and Standards

  • Develops goals for each fiscal year to achieve the hospital and department missions, acting in concert with the directors of pharmacy and the chief pharmacy officer
  • Maintains own professional and specialized competency through continued education: attendance of at least one regional or national meeting per year.
  • Participates and presents/publishes results of research or noteworthy DUE/Quality Improvement projects at local, regional, and/or national meetings.
  • Precepts pharmacy residents as needed
  • Conforms to hospital standards of performance so that the best possible customer service and patient care may be provided.
  • Maintains all hospital information confidential at all times.
  • Follows established hospital infection control and safety procedures.

JOB REQUIREMENTS

EDUCATION:

  • Work requires a Degree in Pharmacy from an accredited college of pharmacy.
  • A master’s degree in a related field (e.g., MBA, MPH, MS in pharmacy) is preferred.
  • Completion of a clinical residency with an ambulatory pharmacy focus is preferred.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED :

  • Work requires licensure to practice pharmacy in the Commonwealth of Massachusetts by the hire date.Applicant can be licensed in another state and pending reciprocation.
  • Board Certification in a specialty area (e.g., BCACP, BCPS) and/or certification in an ambulatory-related clinical specialty (e.g., CACP).

EXPERIENCE:

  • Post-graduate training or at least five years of pertinent work experience.
  • Must have work experience in providing direct patient care (e.g., geriatric medicine, internal medicine, heart failure, infectious disease or anticoagulation) and some leadership experience.

KNOWLEDGE AND SKILLS:

  • Work requires a high level of analytical ability, organizational, and personal management skill to function as a role model and focus to ensure the concerted approach to clinical operations. Must also be able to evaluate current pharmacy services and procedures to improve them through an evaluative process.
  • Work requires a high level of interpersonal skills to relate to hospital healthcare personnel working cooperatively and collaboratively to resolve situations and problems, as well as effectively communicating and displaying leadership within and outside of the department.
  • Work requires a high level of computer skills including word processing; operation of spread sheets and database management.
