Adecco is assisting a Nevada & Placer County Medical group in the immediate search for a Community Health Representative (CHR). This role is to improve the health, knowledge, and well-being of their community of focus by providing outreach, education, navigation, support and advocacy. Working closely with all departments, they are responsible for creating and sustaining meaningful and productive relationships between patients, families, community resources and health care providers. These relationships aim to ensure patients and families understand their conditions and treatment, receive appropriate and timely care, and are connected to community services and other resources available to them.
Responsibilities & Qualifications:

  • Written and oral fluency in both English and Spanish.
  • High School diploma or GED certification.
  • Excellent verbal and written communication and interpersonal skills; ability to maintain high level of confidentiality.
  • Proficiency with accurate data entry in an Electronic Health Record system and Microsoft Suite Products (Word, Excel, Teams, etc.).
  • Knowledge and understanding of the local community and deep understanding and experience with the community of focus.
  • Demonstrated ability to interpret, summarize and report information coherently and logically.
  • Demonstrated ability to establish and maintain effective working relationships; and coordinate activities in the agency and with groups outside the organization.
  • Demonstrated ability to work independently and be self-directed and flexible.
  • Current and in good standing: CDL, vehicle insurance and DMV printout.

Pay range is $23.00 - $26.50
Pay Details: $23.00 to $26.50 per hour
