About Us:

Sonoma Specialty Hospital, as the county’s only Long-Term Acute Care Hospital, is dedicated to caring for patients that require an extended acute care stay due to severe illness or catastrophic injury. We accept patients from the ICU, Step-Down and Telemetry units of partner hospitals and continue their care for as long as 45 days. This care focuses on the challenges of getting patients off of ventilator support, healing severe wounds, aiding their recovery following surgical procedures, and providing rehabilitation to speed their return to daily life. Our Hospital provides ICU, Surgical, and Step-Down care with dialysis, multi-specialty therapies and ongoing planning for the patient’s transition back home or into a lower level of care.

Job Summary:

Responsible for the creative and effective administration of Therapy (Physical, Occupational and Speech) services for the entire continuum of care in accordance with current Federal, State and Company standards, guidelines and regulations with a customer-service focus.

Education, Training and Licenses Required:

  • Minimum Bachelor’s Degree in Physical Therapy or Occupational Therapy.
  • Graduate of an APTA accredited Physical Therapy program or AOTA accredited Occupational Therapy Program strongly preferred.
  • Master’s Degree in Physical Therapy, Occupational Therapy or related health care field preferred.
  • Current California Physical Therapy or Occupational Therapy license required.
  • Current AHA or equivalent BLS certification.

Experience Required:

  • A minimum of 3 years clinical experience in Physical Therapy or Occupational Therapy required.
  • One to two years of demonstrated progressive management experience in a clinical setting with increasing scope of responsibility.

Essential Duties and Responsibilities:

  • Follow Medicare, state and federal regulations, and company policies and procedures and ensure departmental understanding and compliance in the areas of care provision, documentation, billing reimbursement, resident rights, customer service and compliance and other requirements as defined in the respective regulations.
  • Compliant with HIPAA guidelines, provides privacy and maintains confidentiality and ensures residents’ independence and right to make personal decisions to accommodate their needs in accordance with the resident’s interdisciplinary care plan.
  • Consistently and appropriately responds to the needs and concerns of patients, families, and visitors in a pleasant, respectful, and courteous manner.
  • Demonstrates awareness of potential hazards and reports unsafe conditions to appropriate staff to maintain a safe environment for patients, families and staff.
  • Facilitates/participate in hospital and department meetings as appropriate.
  • Work with the interdisciplinary team to achieve/maintain the highest functional level of all customers and promote coordinated, efficient care with a customer service focus.
  • Provide therapy duties in all settings across the continuum of care, as needed.
  • Orient and train new staff as requested.
  • Manage department’s performance by taking appropriate action to ensure the department’s overall compliance with the attainment of goals/objectives, as related to clinical, fiscal, operational and customer service targets. Maintain all departmental reports, statistics and record systems compliant with policy and privacy standards.
  • Responsible for monthly analyses and reporting on assigned business lines’ performance related to defined community/corporate metrics; which include statistics, staffing, budget variances and other items related to success of the businesses managed.
  • Create and execute effective staffing and scheduling patterns to achieve this efficient, cost-effective care, maintain compliance with productivity standards as appropriate.
  • Participate in department/community efforts to provide interdisciplinary meetings/programs that focus on the wellness of residents and the prevention of negative incidents throughout the care continuum.

Knowledge/Skills/Abilities Required:

  • Excellent oral and written communication skills required.
  • Must possess teaching ability, leadership ability, and high professional standards.
  • Must be well organized, be able to set priorities, work independently and is an effective team member.
  • Word Processing skills required. Statistical/data management skills required.
  • Strong knowledge of regulatory processes required (DNV and Title 22).
  • Must demonstrate and maintain current knowledge and skill in providing care for patients of all age groups.

Equipment Knowledge Required:

  • The Rehabilitation Supervisor will demonstrate competency in theory and practical use of all electrical, thermal and mechanical equipment used in this department.

Working Conditions:

  • Hospital and clinic setting with public address system. Patient care equipment noises; Day shift 8:00 to 5:00 M-F with weekend rotation. Teamwork with other professionals.
