Job Title: Intake Coordinator - Hospice
Department: Quality - Hospice
About the Role:
Allied Hospice is seeking an Intake Coordinator to join our Hospice Team! This person will be responsible for receiving referrals into our hospice group from all referral sources, reviewing medical records of prospective patients, completing an intake appointment with prospective patients, and submitting information to the RN for review. The Intake Coordinator will also be responsible for coordinating discharge, coordinating delivery of medications and equipment to a patient's place of service, and assigning the case to the RN for admission. Ideally, we are also looking for someone who can assist our hospice administrator with marketing → you would be working with patients and clients, meeting with case managers, going to doctor offices to ask if they need anything regarding pamphlets/info about our hospice group, and facilitating relationships to create new referral streams.
What You'll Do:
  • Collaborate with members of the intake team, establishing and maintaining positive relationships with patients and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes
  • Directs all daily patient referral and intake operations including providing oversight of the establishment and implementation of intake policies
  • Conduct comprehensive assessments of patients' medical history, current condition, and treatment goals
  • Coordinate and monitor patient care, ensuring that all necessary medical services and resources are provided
  • Monitor and evaluate the effectiveness of treatment plans and make adjustments as necessary
  • Ensures compliance with all state, federal, and ACHC referral/intake regulatory requirements
  • Contributes to the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy
  • Establishes and maintains positive working relationships with current and potential referral sources
  • Ensures seamless transition of patients to home care by providing coordination of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers
  • Ensures maximum third-party reimbursement through oversight of insurance verification and authorization processes
  • Gathers and reports referral statistics including key customer referral trends
  • Contributes to strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs
  • Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources
Qualifications:
  • Medical background, experience in intake coordination
  • Able to multitask, strong communication skills
  • Cantonese and Mandarin Speaking preferred but not required

  • Bachelor's Degree in Healthcare Administration or related field.

Environmental Job Requirements and Working Conditions:
  • This position will take place fully in-office. The office is located at 1680 S Garfield Avenue Suite 205, Alhambra, CA 91801
  • Expected hours are 40 a week, 9 A - 5 P.
  • The total compensation target pay range for this role is: $23 - $26/hr.
