Position: Transition Specialist
Location: Woodridge Interventions | 2221 64th St. | Woodridge, IL 60517
This position works with students, family, staff, and the placing school districts to transition students into Meridian North to prepare students for success in an alternative, therapeutic school. In addition, this position works to transition students back to their home, school and community following treatment.
Salary: $24.50 per hour

Primary Duties and Responsibilities
  • Meet with new students on the first day to help coordinate schedule and intake meeting with school staff.
  • Create a profile of the new student to disseminate information to the staff with regards to student strengths, weaknesses, IEP information, etc...
  • Research, coordinate, and plan fieldtrips to appropriate college and career opportunities for students
  • Research and review with individual student's career and college interests
  • Communicate with college, vocational schools for appropriate student tours
  • Work with students to complete Financial Aide (FAFSA) for future school financing
  • Work with students to answer questions and facilitate future school enrollment and attendance
  • Facilitate potential housing needs.
  • Facilitate the transition of students to their home school or to a lesser level of care after treatment.
  • Provide linkage to the home school and community agencies that our youth will need after treatment.
  • Assist students in the development of a Student Success Plan (SSP), a document that charts the steps and measurements that will determine the effectiveness of the return to the youth's home community.
Minimum Requirements
  • The following educational requirements are acceptable for this classification including:
  • Bachelor in related field and minimum one year of related experience; OR
  • Master in related field and minimum six months of related experience
    • Excellent grammar, communication and organization skills.
Other Qualification
• At least twenty-one (21) years of age.
• Criminal clearances (State Police, Local, FBI, DCFS and National Sex Offender Registry clearances).
• Pre-employment physical exam.
• Valid driver's license.
• Successful completion of a pre-employment drug screening.
• Successful completion of orientation training.

• The employee is occasionally required to reach with hands and arms and use legs to lift/move up to 50 lbs. or more.
• Ability to participate in and supervise therapeutic recreation activities structured for children and adolescents.
• May be required to intervene with and/or physically manage aggressive clients as trained by the Crisis Prevention Institute.

• Ability to work with computers and the necessary software typically used by the department.
