Job Summary
This is a temporary PRN position.
This position provides psychosocial assessment and intervention to patients, facilitates the movement of patients to appropriate levels of care, and refers patients and family members to appropriate resources and services. Most of the patients will be cared for within the dialysis, home health, or hospice setting.
Wage range: $21.87-$32.81

Qualifications
Degree in Social Work and one-year experience in social work setting required.
Licensed Social Worker in the State of South Dakota. Valid South Dakota driver's license required.

Responsibilities include:
- Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of
- significant social and emotional factors underlying patient's health problem.
- Facilitates care planning and helps patient and family through individual or group conferences to understand (omit accept) and follow medical
- recommendations.
- Formulates social service care plans to meet complex psychosocial needs. (omit areas of care)
- Provides service to promote optimum social and health adjustment within patient's capacity.
- Utilizes community resources to assist patient to maintain life in community or to learn to live within limits of disability.
- Prepares patient psychosocial assessments, interventions and goals, and progress reports.
- Participates in planning for improving health services by interpreting social factors and being involved in advocacy efforts for change.
- Teaches and facilitates support groups.
- Provides general direction and supervision to BS/BA social worker.
- Mediates difficult crisis situations.
- Participates within the interdisciplinary team meeting.
- Other duties may be assigned.
