RESPONSIBILITIESI. JOB SUMMARY/RESPONSIBILITIES:• Collaborates with all members of the assigned area’s team and patient’s primary care provider to coordinate the care of the patient across the continuum.• Coordinates all aspects of the pre-operative and post-operative (post discharge) treatment plan/regimen of the program.II. TYPICAL PHYSICAL DEMANDS:• Seeing, hearing, speaking, walking, standing, finger dexterity.• Continuous: lifting to ½ pound, grasping.• Frequent: sitting, bending/stooping below waist level, squatting, twisting, reaching above, at and below shoulders, repetitive arm/hand motion, lifting up to 10 pounds.• Occasional: kneeling, crouching, climbing, walking on uneven ground, horizontal lift/carry up to 75 pounds with assistance, floor to waist up to 35 pounds and pushing/pulling weight to 50 pounds of force.III. TYPICAL WORKING CONDITIONS:• Exposed to chemicals, blood/body fluids and communicable diseases.• Work environment can be fast-paced and hectic.• Not substantially subjected to adverse environmental conditions.IV. MINIMUM QUALIFICATIONS:A. EDUCATION/CERTIFICATION AND LICENSURE:• Current Hawaii State License as a Registered Nurse.• Bachelor’s degree in Nursing required.• Current BLS certification.• International Board of Heart Rhythm Examiners (IBHRE) certification preferred.B. EXPERIENCE:• Two (2) years nursing experience in a hospital, clinic or as a clinical representative for a medical device company.• Experience in program development and coordination of outpatient physician practice.• Experience to demonstrate:o Knowledge of electronic based medical records.o Ability/skills in patient teaching.o Knowledge of performance improvement, database/information systems preferred.Equal Opportunity Employer/Disability/Vet
