Your Role on the Team
Position Summary:
The Social Worker is responsible to facilitate care through resource coordination. This position helps patients achieve access to care and appropriate utilization of resources, balanced with the patients right to self-determination. The individual in this position has overall responsibility for and to assess the patient for discharge planning and transition needs, conduct complex psycho-social assessment and interventions to promote safe discharge preventing avoidable readmissions. This position integrates national standards for case management scope of services and the National Society of Social Workers code of ethics including: Promoting appropriate length of stay while assuring care is the right place and right time; Compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy; Education provided to physicians, patients, families, and caregivers.

Required Qualifications:
  • Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
  • Must demonstrate customer service skills appropriate to the job
  • Excellent written and verbal communication skills in English
  • Ability to maintain a work pace appropriate to the workload
  • Ability to establish and maintain effective working relationships across the Health System
  • Mandatory: EDUCATION: Possess a Bachelor's Degree or Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education (CSWE). Preferred: Strongly prefer at least one to two years acute hospital experience as Social Worker. Physical Requirements: Must have dexterity, coordination and visual/auditory acuity to perform all job functions; Can sit and/or walk for long periods of time. Must be able to function in a busy and distracting environment.
