Summary: The Clinical Director is responsible for overseeing and directing the patient care operations of the home care facility. This includes coordinating and completing assigned projects to effectively support the immediate and long-range objectives of the company; oversight of the eligibility of patients referred to home care services; planning for the services to be provided to patients; and supervising their total home health care. The role also includes implementing and maintaining administrative practices, agency philosophy, goals, and policies which assure compliance with applicable state and federal regulations; enhancing the profitability of the agency; providing motivation and retention of a qualified staff; and ensuring the quality of services delivered. This position acts as a liaison with management staff and other departments throughout the company.

Salary Range: $100K - $130K annually

Responsibilities:

  1. Supervise and ensure high-quality patient care services throughout the episode of care, including referral coordination and the development of individualized plans of care.
  2. Ensure coordination with physicians, families, and other clinicians involved in care.
  3. Communicate effectively with staff to ensure they are informed through staff conferences and mandatory in-services.
  4. Supervise patient care to ensure delivery of safe, economical, and efficient care adhering to relevant standards and regulations.
  5. Monitor and assess agency staff performance regularly using the clinician scorecard and performance evaluation tools.
  6. Ensure that only qualified personnel are hired and oversee licensure verification and credentialing.
  7. Assist with staff recruitment to maintain adequate staffing levels.
  8. Participate as a member of the Management Team and QAPI Team, implementing Performance Improvement Projects and annual strategic agency planning.
  9. Maintain and document an infection control program aimed at preventing and controlling infections.
  10. Conduct monthly meetings with the medical director and approve medical director invoices for timely payment.
  11. Drive operational metrics to ensure the organization meets goals related to operational efficiency, growth, quality, and finances.
  12. Act as Emergency Coordinator in the absence of the Executive Director/Alternate Executive Director during emergencies.
  13. Communicate effectively and professionally with all customers to represent the agency and ensure thorough investigation of concerns voiced by patients, caregivers, and referral sources.
