The Coordinated Family Support (CFS) Specialist provides assistance with setting up and coordinating the services and support needed to allow adult clients to continue living in their family home. This includes developing skills in the home as well as coordinating the delivery of needed services including; helping with “generic services” and helping clients access their community. Support will be provided both in the client’s home and within the community.Essential Duties and Responsibilities :Demonstrates competence in performing the necessary skills and performs those skills listed on the service planUse of phone and email to assist client in researching and setting up services.Communicates plans and progress with the client’s familyMay assist in supervision of client during community outingsProvides support necessary to successfully reside in the family homeProvides assistance and training to the consumer and their family with navigating services and supportAssists with scheduling of service delivery including medical and other appointmentsPrepares and submits quarterly reports on progress of service deliveryStrictly adheres to confidentiality requirementsAppropriately reports changes in client status or as directed by care planMaintains confidentiality of all information pertaining to clients, families and employeesPerforms other duties as assigned/necessaryMinimum Requirements:State licensure or certification as required by state and/or programAssociates level degree in a human services field of study OR 3 years of experience in the developmental disabilities service delivery systemCurrent BLS Card or CPR and First Aid training certificationCurrent PPD or Chest X- RayAbility to coordinate and facilitate services between the home, school, local health agency, and other community resourcesAbility to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language may be requiredComputer proficiency requiredMust be at least 18 years of ageGeneral Comments Pay Rate $16-25About Maxim Healthcare ServicesMaxim Healthcare Services has been making a difference in the lives of our patients, caregivers, employees and communities for more than 30 years.
