This well-established concierge family medicine practice is seeking a full-time Chronic Care Management (CCM) Certified Medical Assistant (CMA) to join its team. The CCM Medical Assistant helps bridge the gap between patient visits, ensuring that chronic care patients receive continuous, comprehensive management of their conditions. In this role, you will contribute to both front and back-office operations to ensure the practice runs smoothly. The ideal candidate will be a team player with strong multitasking skills. Responsibilities of the CCM Medical Assistant include:
Back Office:

• triaging and rooming patients
• facilitating patient flow
• measuring and recording patient vitals in the EMR
• administering immunizations
• drawing labs
• conducting EKGs and PFTs
• returning patient calls
• securing prior authorizations
• manage referrals and prescriptions
• other duties as requested

Front Office:

• greeting patients
• check-in/check-out
• scheduling appointments
• taking co-pays
• answering phones
• managing referrals
• managing prescriptions
• entering demographics
• other general office duties as required

Chronic Care Management:

• Provide patient education on managing chronic conditions
• Track patients vital signs, symptoms, and medication adherence
• Regularly check-in with patients between appointments to ensure they are following their care plan, addressing any concerns, and adjusting treatment as necessary
• Manage referrals
• Manage medications

Applicants must have:

• A minimum of two years of medical experience, preferred
• Experience with chronic care management
• Proficient computer and EMR skills
• Strong communication and organizational skills
• Outgoing and friendly personality
• Familiarity with medical billing
• Ability to thrive in a fast-paced environment
