Columbine Poudre Home Care's office is located at 915 Centre Avenue, Suite 2, Fort Collins, Colorado and is a locally owned agency providing home care and health education. We pride ourselves on providing high quality, efficient, convenient and cost-effective health care in the homes of our patients. Home care requires a team effort involving professionals, paraprofessionals, patients, and their families.
We are currently accepting applications for a quality Medical Social Worker able to work:
- PRN
- Monday to Friday daytime hours with occasional evenings and weekends
Medical Social Worker
- As a Home Health Social Worker, you will meet with clients and their caregivers in their homes providing direct support during times of transition.
- You will assess the social, emotional, and environmental factors that are impacting their medical condition and recovery, and counsel them for long-range planning.
- You will educate for community resources, providing guidance on how to access the right support at the right time to support their long-term safety and well-being.
- Your skills are crucial at times of crisis intervention. We look to you to decrease potential for abuse or neglect, strength fragile support systems, and reduce risk of harm.
Requirements
- Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education.
- Valid license to practice in CO.
- One year of social work experience in a health care setting.
- Must have and maintain a valid driver's license, maintain automobile insurance coverage and have access to an automobile.
COMPENSATION
Starting wage $31-$36/hr. per with travel reimbursement.
