We value work-life balance and offer an extensive benefit package.We are currently hiring a Code Blue Case Manager to work in our Delaware, Otsego & Schoharie Counties agency. As the Blue Case Manager, you would be responsible for:Temporary for the Warming Station Season (11/1-3/31)Provide case management services to homeless individuals at the Warming Station, including intake, assessment, referrals, and service plan creation.Establish regular contact with each household, either in person or by phone, to monitor progress and address barriers to goal achievement.Offer direct services such as advocacy, coordination, referrals, transportation assistance, budgeting, and financial support related to housing.Maintain up-to-date case files, document all case management activities, and track grant funding agreement targets.Collaborate with Catholic Charities and other agencies, attend meetings and trainings, and ensure the confidentiality of client information in accordance with agency standards.Blue Case Manager Minimum Salary: $30/hrBlue Case Manager Maximum Salary: $30/hrBlue Case Manager Schedule: M-F at the Warming Station from 5-10:30 pmQualifications:Associate’s Degree with 3 years of experience preferredHS Diploma and 5+ experience required.Catholic Charities is an Equal Opportunity Employer.Applicants have rights under Federal Employment Law:Family and Medical Leave Act (FMLA) ( Employment Opportunity (EEO) Know Your RightsEmployee Polygraph Protection Act (EPPA) (
