ESSENTIAL DUTIES AND RESPONSIBILITIES
Sensitivity to the cultural diversity of others
Understands the medical, psycho-social, educational characteristics and needs of the population served by the Agency
Thrives working independently and on a team
Provides outreach and enrollment, develops Plans of Care and Individualized Family Service Plan (IFSP), and conducts continuing care management services with children and families
Coordinates, identifies, and secures services relevant to the Plan of Care and Individualized Family Service Plan (IFSP)
Completes documentation with a high level of quality and detail to meet and exceed Health Home, Early Intervention and Department of Health requirements
Participates actively in trainings, meetings, and functions at the agency and in the community
Provides quality direct Care Management services to children and families in home, school, and community settings
Provides quality Ongoing Service Coordination for children birth to three (3) years old who are dually eligible for Early Intervention and Health Home services.
Uses a strength-based approach to arrange for, educate about, and manage the coordination of medical care and other services as needed
Provides on-call availability to Health Home Program as assigned
Interfaces and networks with partnering Health Homes, providers, and agencies
Must obtain certification to administer CANS-NY within 30 days of employment; certification must be continuously maintained while in the position
Provides transportation to children and /or families to appointments (i.e. medical appointments, Plan of Care meetings, DHS, community agencies, etc.) as needed.
Other duties may be assigned.

EDUCATION and/or EXPERIENCE
Bachelor's degree in Human Services, Nursing, or related field required; Master's degree in Human Services or Nursing preferred, and a minimum of one (1) to three (3) years related care management experience with persons with disabilities and/or chronic illnesses.
