We are seeking a Behavioral Care Manager to join our team. The ideal candidate will be responsible for coordinating behavioral health care for our patients and collaborating with primary care providers. This position offers the opportunity to work remotely and play a key role in delivering high-quality mental health care.

Responsibilities

• Coordinate behavioral health care with primary care providers
• Conduct screenings and assessments for mental health and substance abuse disorders
• Monitor patient progress and treatment effectiveness
• Educate patients about mental health and treatment options
• Develop and implement care management plans

Requirements
