Case Manager Outpatient Safe Haven (Full Time)
The Safe Haven Outpatient program is comprised of a multi-disciplinary team that includes not only the case manager, but a Team Leader, prescriber, nurse, housing specialist, and peer recovery specialist. This Team works closely with one another to coordinate care for some of the most vulnerable members of our community: individuals with co-occurring mental health and substance use, many of whom are homeless. You are a great fit for this team if you are mission-driven, person-centered, and trauma-informed. Must have tremendous compassion for others while maintaining excellent boundaries. Extensive knowledge of the resources available within our community. Open to training on agency resources, as well as evidence-based practices.
Strong verbal and written communication skills are a must, including documentation of treatment plans and progress notes in the Electronic Health Record. Demonstrate good time management and ability to multi-task. Ability to notice changes in client mood and level of functioning in order to engage other members of the care team to respond as necessary. Willing to go out into the community, including encampments (with assistance from another team member), to meet clients where they are at. Must have a valid Driver's license and be willing to transport clients.
Job Responsibilities:
- Work with clients to develop a treatment plan, under the supervision of the Team Leader.
- Support delivery of services outlined in the client's treatment plan.
- Assist the client in scheduling and attending appointments with the psychiatric provider.
- Assist clients in accessing benefits/entitlements.
- Assist clients to access basic needs.
- Connect client to additional CCA and community resources, including but not limited to education, employment, and housing.
- Coordinate care with primary care and medical specialists to schedule and keep appointments, and adhere to the recommendations of these providers.
- Connect clients with natural and social supports.
- Assist clients to navigate legal issues, when relevant.
- Support the client in making healthy lifestyle choices regarding sleep, diet, and exercise.
- Connect clients to higher levels of care as needed and support them in stepping down from these.
