As a Housing Coordinator within the Project Impact program at the East Central Public Health District, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.

Job Responsibilities
* Acts as client advocate in order to coordinate required services or to resolve crisis
* Counsels' clients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life
* Identify environmental impediments to client of patient progress through interviews and review of patient records
* Monitors, evaluates, and records client progress according to measurable goals described in treatment and care plan
* Organizes support groups or counsel family members to assist them in understanding, dealing with and supporting the client
* Performs clinical assessments of the behavioral health needs of consumers presenting for services
* Provides case management services related to identified treatment needs
* Provides training to direct care staff in implementing behavior modification techniques and treatment plans
* Refers client or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education
* Receives additional training, as required, to gain full proficiency and experience in all areas
* Works closely with clients, landlords and community organizations.
* Conduct face to face intake assessment to determine housing needs & eligibility for program.
*Provide ongoing support and case management to help clients achieve and maintain stable housing.
* Establish and maintain positive relationships with landlords and property managers.
* Collaborate with community organizations, social services, and other agencies to connect clients with additional resources such as nutrition, financial assistance, employment support, and healthcare.
* Attend mandatory events such as: Augusta Pride, National HIV Day, and World AIDS Day.
* Assist with monthly events if needed.
* Take the HIV Counseling and Training class.
* Need reliable transportation.
* Maintain accurate records of client interactions and services provided.
* Ensure compliance with all relevant regulations and funding requirements.
* Provide support and intervention during housing crises, including eviction prevention and emergency housing placements.
* Participate in community outreach and education efforts to promote housing stability.
* Attend required HOPWA trainings in and out of state.
Minimum Qualifications
* Master's degree in a related field from an accredited college or university AND Eligibility to be licensed
* Or bachelor's degree in a related field from an accredited college or university AND Two years of years of experience in social service delivery.
* Or 5 or more years' experience with High school diploma and/or associate degree.
Preferred Qualifications:
* Excellent verbal and written communication skills.
* Excellent computer skills, experience with electronic medical records a plus.
* Experience with CAREWare/HMIS a plus.
* Ability to maintain HIPAA compliance, corporate compliance and client confidentiality.
* Collaborate with community organizations, social services, and other agencies to connect clients with additional resources such as nutrition, financial assistance, employment support, and healthcare.

DPH accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered. DPH will contact educational institutions to verify degree, diploma, licensure, etc.
