Provides for the delivery of services to the residents of Sheridan Springs and Pine Meadows. Primarily responsible for the development and management of the resident treatment planning process. Ensures communication and contact between the treatment program and residents' families. Provides crisis prevention training to all employees of the residential facilities.
WHAT YOU'LL DO:
- Maintain contact between the treatment program and residents' families or other professionals involved.
- Serve as therapist in individual and group therapy.
- Responsible for development and implementation of resident treatment plans in collaboration with the Interdisciplinary Team.
- Identify the resident's level of functioning and need for services in a variety of areas as well as the development of specific interventions and treatment strategies to address those needs.
- Ensure that services provided to the resident are appropriate to address both short-term transitional or long-term residential needs of the resident.
- Regularly evaluate resident progress toward treatment goals.
- Establish and facilitate programming that provides maximum benefit to the residents.
- Write resident social histories and develop the Functional Needs Assessments for all residents upon admission and annually thereafter.
- Complete monthly progress notes reflecting resident progress, significant events, and contacts.
- Assist residents with financial accountability and planning.
- Ensure residents are accessing all benefits they may be eligible for and assist residents in applying and re-applying for benefits and services as needed.
- Participate in the facilities' staff development and training program.
- Coordinate referrals for services upon discharge and other after care needs.
Qualifications:
Education:
- Bachelor Degree in Social Work required; or Bachelor Degree in a related social services area (defined as including Sociology or Psychology) with at least 30 semester hours in social work (twelve months of experience in the delivery of related social services in a social service or health field may be substituted in lieu of classroom training).
Skills and Experience:
- At least two years experience working in a residential setting required.
- Experience working in a state licensed residential care facility preferred.
- Computer competency including use of spreadsheets and word processing applications (MS Office) and electronic health record systems preferred.
- Ability to effectively function in a team environment.
- Attention to detail and possess excellent written & verbal communication skills required.
- Possess excellent organization and time management skills.

authorize the release of this information by the appropriate agencies to the investigating service. I authorize and give permission for
the individuals listed as references to provide written information and/or verbally discuss my background with Vera French
Community Mental Health Center, or any of its affiliates.
This authorization, in original or copy form, shall be valid for this and any future reports and updates that may be required.
I understand that passing the background check is a condition of employment. A negative background check can be grounds for
dismissal, even if an offer has been made to me and I have been hired.
