Overview

The Regional Clinical Director of Abode Care Partners Primary Care leads the clinical operations, programs, and activities for the assigned region and ensures the delivery of high-quality patient care. This includes services spanning Skilled Nursing Facilities, Assisted Living, Group Homes, Independent Living, and private residences. This position is a key member of the Clinical Operations leadership team and will advise on and execute strategy for the company in partnership with all partner team resources including professional development, human resources, quality and coding, and value-based operations.

Responsibilities

For assigned territory:

  • Lead and manage up to 30 Advanced Practice Providers as direct reports.
  • Responsible for on-boarding and care model training of new Advanced Practice Providers.
  • May provide clinical coverage for unexpected call-offs, vacations, or special needs as indicated and required.
  • Conduct professional development and the annual performance, competency, and compliance review process.
  • Drive performance and support retention efforts to include regular coaching and review of:
    • Provider incentive structure.
    • Provider scorecard (coding, quality, patient experience, and productivity reporting) reviews.
    • Visit distribution and workflow improvement.
    • Performance Improvement Plan creation and management.
  • Execute on regional business, growth, and value-based strategies for respective operations.
  • Provide account and relationship management in conjunction with the regional administrative partner: increase building penetration, serve as the escalation for the leaders for all facilities in your region, and engage with patients and/or families to facilitate issue resolution.
  • Provide outstanding customer service to all clients, patients, and families.
  • Proactively problem solve with all key partners to support retention, recruitment, and operational efficiency.

Qualifications

  • Master’s degree in Nursing.
  • Advance Practice Provider: APRN, FNP, AGNP or PA.
  • At least three years of experience practicing in Assisted Living, In-Home, Group Homes, Independent Living, or Skilled Nursing Facilities.
  • At least two years of nursing management experience in a health care industry.
  • Strong interpersonal and communication skills to effectively communicate with Practitioners, office personnel, and patients.
  • Demonstrated ability to develop and implement clinical initiatives.
  • Proven track record of improving patient outcomes.
  • Computer literacy and knowledge of relevant healthcare and administrative software.
  • Excellent analytical and problem-solving skills.
  • Organizational and time management skills.
  • Strong leadership, motivation, training, and goal-setting skills.
  • Strong financial acumen and budget management skills.
  • Ability to travel up to 30% in assigned region.
