This position is an on-site position located in North East Portland at a women's substance use residential treatment program. Children between the ages of newborn to 9 years old are also residing on-site with their parent.

Care Coordinator provides a wide range of clinical and administrative services to improve client care within a women's substance use treatment residential setting; to assist in the coordination and linking of clients to medical and psychiatric services; works to optimize service delivery by assisting with and facilitating improved access to clinical care across agency programs as part of a strengths-based, consumer-centered approach.

Location: This position is based in Northeast Portland.

  • Stabilize women who are seeking residential recovery treatment so that they enter and stay in treatment.
  • Help women successfully transition to outpatient care from a residential treatment setting.
  • Support reunification of mothers and children.
  • Support 140 women annually through residential program entry with quick access to comfort medications, prescription refills, mental health care, a PCP, and other needed supports.
  • Support the women on program exit with connections to methadone clinics, prescription needs, and crisis response.
  • Support up to 50 children annually with prescription access, psychiatric care, speech therapy, medical equipment, and other needs.
  • Build and maintain connections to expand partnerships for methadone clinics and other resources for consumers on program exit. Must have connections to MAT providers.
  • Using information provided by a variety of sources, identify LifeWorks NW clients with chronic health conditions (e.g. diabetes and SPMI) that are inadequately engaged with physical and oral health providers.
  • Work with inter-disciplinary team to promote coordination of service delivery and prevent duplication of services.
  • Coordinate referrals with other providers in a timely and professional manner. Collaborate with inter-disciplinary team and community services to ensure that any diseases, as well as preventative and health promotion needs, are addressed as part of the holistic, whole-person, culturally-sensitive approach.
  • Assist identified individuals with accessing needed physical and oral health care by bringing their situation to the attention of their treatment team and attending clinician.
  • Assist staff working with identified individuals with overcoming any barriers impeding access to physical and oral health care and supporting clients in maintaining health.
  • Track referrals and their status on identified metrics to determine project effectiveness.
  • Develop relationships with appropriate staff at medical and dental providers to assist treatment teams and attending practitioners help individuals access needed services and coordinate care.
  • Identify care systems and services to assist in coordination of care with appropriate partners such as primary care providers, oral health providers, or other medical providers in consultation with team and supervisors.
  • Provide direct care coordination, skills training (i.e. improve functioning and independence), and case management services.
  • Maintain confidentiality of records relating to clients' treatment.
  • Meets standards for documentation and accuracy of work as defined in the Performance Evaluation.
  • Meets standards as defined in the Performance Evaluation for client care, quality/customer service, attendance/punctuality, and teamwork.
  • Continually increase level of cultural sensitivity, awareness, and competency.
  • Participates in regular individual and team meetings with supervisor. Provides written and verbal reports to supervisor regarding cases. Attends informational and clinical meetings, which may include all-agency meetings, team meetings, consultation meetings with psychiatrist, staffings, intra-agency trainings, etc.
  • Checks e-mail/voice mail daily/throughout the day when working responding to contacts within 48 hours for routine matter (urgent matters may require a more immediate response).
  • Completes online training courses as assigned.
  • Travels between sites or in the community when needed.

Qualifications:

  • Can demonstrate experience and proficiency working with culturally-focused programming for African American women.
  • Required: The incumbent must demonstrate the competencies as outlined in OAR 309-016-0005 to become credentialed as a Qualified Mental Health Associate (QMHA).
  • The CADC credential is preferred, given the high occurrence of substance use/abuse, however not required.
  • Preferred: Clinical experience providing outpatient services or working in community health, primary care is preferred.
  • Preferred access to a car, a valid driver's license, proof of current automobile insurance coverage, and verification of safe driving record from DMV.
  • Demonstrate effective communication skills and ability to provide culturally-competent treatment services to diverse client populations.
  • Ability to perform essential functions of job without creating a direct threat to the safety of self or others.
