**Must-Have:**
- Ability to work 1 weekend shift per contract
- Must have own transportation
- Experience in skilled home health
- Willingness to adhere to floating policy between facilities within the state
**Job Description:**
- Provide physical therapy services in home health settings across various areas.
- Manage a caseload and complete documentation in EPIC.
- Typically visit up to 6 patients per day, working towards a standard of 30 productivity points.
- Work with minimal drive and windshield time as clients are grouped efficiently.
- Support provided for required communication devices if not supplied.
- Completion of non-billable modules required pre-start and annually.
- Orientation includes 40 hours, not billable.
**Additional Information:**
- Open to local applicants within a 50-mile radius.
- Previous experience with Oasis documentation is preferred but not required.
- COVID-19 vaccine required; medical/religious exemptions and declinations allowed.
