Support patients, and whomever they define as family, articulate health care goals, and provide information and referrals for emotional/spiritual resources as needed.
Advocate for goal-concordant treatment and care for patients and facilitate advance care planning.
Connect patients and families with hospice or other palliative care programs (if appropriate).
Manage, organize, and update relevant data using database applications.
Communicate effectively and efficiently with patients, caregivers, and interdisciplinary care team members.
Participate in Quality Improvement projects.

Minimum Qualifications:
A bachelor's degree in health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
Possess a valid licensed Clinical Social Worker (LCSW) certification in the State of Colorado.
A bachelor's degree in health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
One (1) year of relevant professional healthcare experience.
Possess a valid licensed Clinical Social Worker (LCSW) certification in the State of Colorado.
A bachelor's degree in health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
Two (2) years of relevant professional healthcare experience.
Possess a valid licensed Clinical Social Worker (LCSW) certification in the State of Colorado.
A bachelor's degree in health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
Three (3) years of relevant professional healthcare experience.
Possess a valid licensed Clinical Social Worker (LCSW) certification in the State of Colorado.
Applicants must meet minimum qualifications at the time of hire.
Preferred Qualifications:
A master's degree in health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
One (1) or more years of palliative care experience.
Knowledge, Skills, and Abilities:
Ability to communicate effectively, both in writing and orally.
Ability to establish and maintain effective working relationships with employees at all levels throughout the institution.
