Description
POSITION DESCRIPTION: To Engage, Assess, and Assist eligible participants matched through CES by collaborating with Center consumers to build independent living skills, access needed resources, and make the transition from homelessness to permanent housing.The housing navigator will work as part of a larger team within the Coordinated Entry System for Single Adults of Service Planning Area (SPA) 2. JOB ID#35247A
POSITION RESPONSIBILITIES: Provide direct and indirect client services.
Make copies of all housing documents (vouchers, leases, applications, etc.) before developing & maintaining housing file, which is to be reviewed on a quarterly basis.
Document services provided in Clarity (case notes, file uploads, assessments, referrals, etc.) for each assigned case.
Complete Enrollment, Status Update, and Annual Assessments in Clarity adhering to LAHSA guidelines.
Enroll and Exit housing navigation clients adhering to LAHSA guidelines.
Research and locate available housing units by conducting on line searches and by reaching out to owners that have accepted HACLA offers in the past.
Develop and maintain professional relationships with property owner, managers, and representatives by making regular outreach efforts.
Transporting clients to and from available units, HACLA appointments, LACDA appointments, and services needed to access permanent housing.
Adopt a teamwork philosophy and collaborate with fellow housing staff and case managers from across the agency.
Complete VI-SPDATs, CES Referral Packets, and CES Revision Tools for all clients across the Agency as assigned by management.
Complete COC/S8/EHV applications for clients matched to resources as assigned by manager.
Attend Trainings, Meetings, and Consultation opportunities to improve knowledge of housing systems and improve on understanding of hosing resources, policies, and procedures.
Qualifications
Education: High School diploma or equivalent is required for this position.
Experience: Minimum of two years experience in mental health setting with mentally disabled population preferably with homeless individuals, conducting individual and group counseling.
Experience in advocacy with the homeless mentally ill population.
Ability to speak Spanish is desirable but not required.
Good driving record is required and must be insurable by Center's insurance company.
Must be comfortable working with a target population of homeless, mentally ill, and those suffering from substance abuse and other disabling conditions.
