The Norfolk Community Services Board (NCSB) is the primary provider of public mental health, substance abuse, and intellectual disabilities services for the City of Norfolk. A continuum of services spans from outreach and prevention through intensive psychiatric care, provided by highly clinically oriented service professionals with expertise in these service areas.
Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. The NCSB is seeking a Co-occurring Case Manager III - Discharge Planner. The Co-occurring Case Manager III - Discharge Planner will coordinate with MHCM, ACT, and other departments whose clients are found to be inpatient in the "Point Click Care" portal (inpatient hospitalization portal). Will work with these clients and CSB providers to ensure stability and support is provided to clients discharging from the hospital. Provides services and resources to clients being discharged to prevent recidivism and rehospitalization. Completes clinical documentation in accordance with agency guidelines.
Departmental Hiring Salary Range: $46,589 - $53,577
Essential Functions
Essential functions include but are not limited to:
  • Review inpatient hospitalization portal daily to determine which clients are receiving CSB services and require discharge planning support. Contact the various CSB departments where the client is hospitalized to inform them of their admission. Contact the hospital to coordinate with the discharge planner/social worker and began the discharge planning process. Ensure active ROI in place for the hospital.
  • Send a request for medical records as soon as the client is out of the hospital to have in place for their post hospitalization discharge appointment.
  • Visit the Community Support Services' clients in the hospital within a few days of admission to check on them, begin discharge planning, and coordinate their care.
  • Coordinate with the case managers from the various departments to determine how to optimally assist the client with discharge planning without overlapping services.
  • Link the client to services, resources, and supports to support them prior to discharging from the hospital. Examples include linking to entitlements, substance abuse treatment, medical care, food, housing, etc.
  • Follow the discharge recommendations from the hospital and link client to services needed (i.e., MHCM, ACT, MHSS, PSR, etc.).
  • Assist the individual directly with obtaining needed resources. Assistance also includes advocating for the client to procure needed resources or services, while empowering the individual to be as independent as possible. Obtain ROIs as needed.
  • Contact family or significant others to promote support and stability of client discharging from the hospital.
  • Provide practical counseling and education to guide the client in problem solving strategies and to develop a supportive therapeutic relationship. This may include encouraging compliance with treatment, substance abuse education, discussing relapse prevention strategies, stress management, etc. Performs crisis intervention to resolve critical situations.
  • Manages a small caseload of ten co-occurring clients.
Education/Experience
Bachelor's degree in a human services field.
Two years experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations.
Preferred Experience:
  • Experience in mental health, to include work in case management and experience with individuals with serious mental illness and substance abuse disorders.
Additional Information & Requirements
  • Valid Driver's License may be required depending on assignment.
  • Certification in basic first aid and CPR, depending on area of assignment.
  • Signing Bonus: This position is eligible for a one-time $5,000 signing bonus for new hires.

If you are hired October 5, 2010, or after to a position with City of Norfolk Retirement System benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.
If you are hired January 1, 2022, or after to a position with Virginia Retirement System (VRS) benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.
  • The Tuition Assistance Program is established to encourage employees toward continued self-development and education.
