Job Description
Join an interdisciplinary team, as the Social Worker, in the Comprehensive Psychiatric Evaluation Program (CPEP) Crisis Outreach Program, commonly referred to as Mobile Crisis Team (MCT).
- MCT is responsible for evaluating and assessing individuals in the community at risk of psychiatric hospitalization. Referrals are generated through inpatient units, outpatient programs, NYC Well and other referral channels within the hospital.
- They will conduct assessment, de-escalation, stabilization, referral to community based behavioral health and other support services for individuals who are experiencing behavioral health crisis and to avoid unnecessary interaction with police, ambulance and hospitalization as much as possible.
- The clinician will use a trauma informed person centered, recovery-oriented approaches to support the individuals' capacity to manage their health & wellness, live a self-directed life, and reach their fullest potential.
- The clinicians goes into the community in at least a pair of 2 individuals and conducts the initial psychiatric assessment, documenting biopsychosocial assessments, formulation and disposition, facilitating supportive, psychoeducational and insight-oriented psychotherapeutic interventions, providing crisis interventions to high-risk patients, engaging with and providing support to families, and engaging them as partners in recovery. Documenting all interventions within the required regulatory timeframes is expected.
- They will be responsible for Identifying and collaborating with existing support systems, facilitating community referrals as needed, collaborating with interdisciplinary treatment team to develop most appropriate case disposition, documenting all interventions within the required regulatory timeframes.
- They will also be required to facilitate a removal from the community for someone experiencing a psychiatric crisis.
- When needed, they may also be assigned to provide coverage to the CPEP by completing evaluations and re-evaluations.
- Good writing skills and ability to work in a fast-paced environment required with effective time management skills is necessary.
- Candidate must be open to working primarily in the field/community.
- Tour: Rotating (7 days a week, all holidays, 8am-8pm)
For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
