Job Title: Patient Navigator

Part time, 0.5 FTE, 20 hours per week. Day Shift.

POSITION SUMMARY: The navigator helps guide the patient through the healthcare system and works to overcome obstacles that are in the way of the patient receiving the care and treatment they require. The navigator will coordinate the collaboration with other healthcare providers on patient Care Team to ensure all parties are working in partnership on their mutual patient.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Orchestrate collaboration time with Care Team to ensure all parties are available to discuss mutual patient as needed.

  • Facilitating communication between the patient, family members, and healthcare providers to ensure patient satisfaction and quality of care.

  • Ensuring continuation of care and linkage to community resources as needed.

  • Informing the patient on financial aspects of care and linking patient to financial resources as needed.

  • Daily review of next day provider schedules to gather information necessary for morning care team huddle.

  • Aid Practice Manager in weekly team collaboration huddle with in- office providers to discuss patients seeking or receiving care from multiple providers within the practice.

  • Serves as the first-line resource for all customers (internal/external) with regards to Insurance and Referrals. Must be knowledgeable in all areas of insurance verification and authorization/referral processes.

  • Serve as liaison for patient with doctor offices with regards to processing referrals. Communication may be via fax/telephone/electronically.

  • Liaison between patient and insurance company to explain to patient what their insurance company requires to meet referral/authorization compliance.

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

  • Maintains knowledge of registration and scheduling processes with the capability of filling in as needed based on office workload or staffing demands.

  • Other duties as assigned.

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

  • High School Diploma or General Equivalency Degree (GED).

  • Three (3) years of clerical, customer service, or administrative support experience in a highly customer-oriented organization.

  • Three (3) year experience with basic keyboarding, personal computer use, and other office setting equipment.

  • One (1) year of registration experience, point of service collection, insurance validation, understanding of compliance /regulatory guidelines and order release process(es).

  • One (1) year prior experience of Revenue Cycle in a Hospital/Medical Office Setting.

PREFERRED QUALIFICATIONS:

  • Associates or equivalent experience in health care preferred.

  • Previous Epic or equivalent Electronic Medical Record experience.
