JOB TITLE : Case Manager
DIVISION : Tribal Administration
DEPARTMENT : Tribal Housing
FLSA STATUS : Non-Exempt
SUMMARY : Under the direction of the Housing Coordinator this position will primarily be responsible for assisting clients with securing permanent and affordable housing along with providing a full range of case services through coordination and advocacy for clients participating in the Tribe's housing program.
ESSENTIAL DUTIES AND RESPONSIBILITIES : Including but not limited to the following duties and responsibilities.
  • Conducts client initial intake and comprehensive assessments to determine eligibility and level of assistance
  • Provides support and guidance on securing and retaining housing including gathering and assisting with completing necessary paperwork
  • Creates individualized case plans with measurable goals and objectives
  • Documents, maintains, and prepares confidential records
  • Reports regularly with Supervisor on participant's status
  • Refers participants to other services such as supportive services, mental health and substance abuse; manages crisis interventions
  • Educates participants in subjects that cover home budgets, rental leases, tenant rights and responsibilities, eviction prevention, and life skills
  • Assists participants with home searching activities including viewing homes, walk throughs, and performs home visits
  • Generates quarterly needs assessment for participant's financial and supportive needs
  • Identifies barriers and challenges in obtaining and sustaining housing and well being
  • Networks and collaborates with outside agencies and community-based organizations
  • Communicates with Tribal Directors within the organization as needed to implement programming and services for participants
  • Directs the preparation of a variety of narrative and statistical reports
  • Assures that all department employees adhere to applicable Tribal, Federal, State, and local laws, regulations, standards, and policies
  • Transports participants per program guidelines
SUPERVISORY RESPONSIBILITIES :

KNOWLEDGE OF :
  • Case management
  • Housing for the homeless and/or are at risk of homelessness
  • Working with people who have complex health and behavioral health issues
  • Community resources related to housing and how to obtain assistance
  • Leases, contracts, evictions and other housing related documents
  • Program planning and development practices
  • Federal, State, and local laws and regulations applicable
  • Financial management in programming, funding, and grant contracts
ABILITY TO :
  • Apply cultural sensitivity to cultural relevant programming, primarily Native American
  • Communicate with clients of various backgrounds with a strong sense of customer service
  • To manage crises, de-escalate, empathize and remain calm under intense situations
  • Demonstrate leadership skills, including negotiation, problem solving, and decision making
  • Establish and maintain cooperative and effective working relationships with the staff, community groups, and other related agencies
  • Communicate effectively both orally and in writing
  • Operate standard office and classroom equipment including a computer and assigned software
  • Observe and follow health and safety regulations
  • Maintain files
  • Work with minimal supervision on assigned duties
Education and Experience Requirements:
Associate's degree in a Behavioral Science, Social work, Sociology, Human Services or Psychology and minimum two (2) years in modality of alcohol and drug treatment facility or in mental health or domestic violence counseling OR
High school diploma or equivalency certificate GED/HiSet and four (4) years of related work experience and minimum two (2) years in modality of alcohol and drug treatment facility or in mental health or domestic violence counseling
CERTIFICATES, LICENSES, AND REGISTRATIONS : To perform and to continue to be qualified for this position, the requirements listed below must be met at a time of hire or may be obtained within a specific amount of time depending on the position requirements. Costs for required certifications, licenses, and registration are to be covered by employee, unless offered by the Tribe.
  • Prior to hiring and throughout employment must possess a valid California Class C Driver's License and maintain insurability under the Tribe's vehicle insurance policy
  • Requires driver to be at least 21 years of age in order to transport minors
  • Prior to hiring and throughout employment must maintain regular contact with or control over Indian children and is therefore subject to the background investigative process to comply with P.L.101-630
  • Requires throughout employment maintaining a CPR/AED/First Aid Card or Certification
  • Mandated reporting training required and maintained throughout employment
