Wages & Benefits

Schedule

This position will be located primarily in schools doing outreach with typically one to two days per week in an office setting.

A Typical Day in the Life...

A typical day as Sabathani's Youth Housing Coordinator provides an opportunity to provide professional, compassionate, and high-quality case management and assistance to youth experiencing homelessness within the Minneapolis and neighboring school districts. Responsibilities will include the following:

  • Providing case management to youth in alignment with their goals towards sustainable
  • Completing intakes and assessments for new youth participants.
  • Providing youth with referrals to community resources and services.
  • Documenting the following in SalesForce: contact, enrollment, services, referrals, case management hours, and case notes.
  • Creating a safe and supportive environment for all homeless youth.
  • Providing guidance and support to school personnel, Social Workers, and Homeless Liaisons.
  • Maintaining safety policies and procedures within outreach locations.
  • Attending and representing Sabathani at various community meetings.
  • Assisting with the facilitation of and transportation for weekly support groups.

Who You Are

  • You have professional or related lived experience in providing case management for at-risk homeless youth ages 16-24 of at least 3 years.
  • You are able to work effectively in a diverse, mission-driven agency committed to the principles of diversity, equity, and inclusion with respect to race, ethnicity, gender identity, sexual-orientation, socio-economic status, nationality, and religion.
  • You have the ability to establish and maintain effective relationships with youth, families, and community service providers.
  • You demonstrate a clear understanding of effective crisis intervention and programing that is trauma informed, strengths-based, client-centered, and follows a harm reduction philosophy.
  • You are proficient in MS Word and Excel, and you can produce letters, emails, and reports of professional quality.
  • It is preferable if you have experience using data and working with a database.
  • You are highly responsive with a positive attitude and the ability to work cooperatively with different types of people.
  • You have the ability to communicate in writing and verbally in English; a secondary language is preferred.
