Functions as part of an interdisciplinary team of physicians, nurses and other professionals to maintain continuity of patient care for all patient populations from admission to discharge. Participates in developing and implementing programs and initiatives.ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:Completes a comprehensive psychosocial assessment of patients and reports findings to families, physicians and other healthcare providers as appropriate.Facilitates social service contacts and develops treatment plan in coordination with social service resources.Facilitates family and relational counseling in relation to social, psychological, financial and other issues which are present upon admission or as part of the discharge planning processAssists patient and family to understand and carry out medical recommendations and solve problems that interfere with treatment care plan.Assists patients and families in areas of resource planning.Develops and maintains relationships with a wide variety of community agencies and facilitates communication between these agencies and patients, families and clinical personnel.Acts as a patient advocate. Investigates and reports adverse occurrences, and performs staff education related to resource utilization and psychosocial aspects of healthcare delivery.Identifies psychosocial issues that are present and provides recommendations for treatment and care after discharge.
