Sentara Healthcare is seeking a Full Time/Day Shift Transplant Coordinator for our Kidney/Pancreas Transplant team at Sentara Norfolk General Hospital.

Position Highlights:
Innovation: 525-bed Level 1 trauma center and tertiary care facility that was first in the region to perform kidney, heart, and pancreas transplants.
Market: Sentara Transplant Center has been around for nearly 40 years and is the only transplant center in Hampton Roads Region. We have performed more than 2,200 transplants.
Nursing Excellence: We were the first hospital in the region to become a Magnet recognized hospital.
What you will do:
  • Coordinate care with post-transplant patients.
  • Facilitates clinic flow, transplant evaluation, and patient care management.
  • Provide weekly education sessions to patients and support member. Arranges appointments with surgeons, social worker, financial coordinator, pharmacy and dietary.
  • Transplant Coordinator admits patient, provides support to patient/caregiver during the waiting period, and discharges transplant patient with instructions for going home.
  • Transplant Coordinator will manage a case load of patients, remain patient's point of contact through the transplant process, recovery and at home.
  • Registered Nurse, BSN should have 2+ years of acute care experience, good medical - renal knowledge, and understands chronic disease. BLS required. Certified Clinical Transplant Coord (CCTC) required within three (3) years of hire.

Outpatient clinic is open Monday - Friday between 8am and 4:30pm. Transplant Coordinator will have some scheduled call and have weekend call about every 11th week.

Job Summary
Coordinates the clinical management of the transplant patient including the transplant evaluation, the transplant process and follow-up care. Provides clinical leadership to care team members, allowing staff to plan, direct, manage outcomes and evaluate care.
Qualifications:
BLD - Bachelor's Level Degree: Nursing (Required)
