Job Summary: We are seeking a compassionate and skilled Social Worker to join our team. The ideal candidate will have 2-3 years of experience in social work, preferably within a healthcare or senior living environment. This role involves collaborating with residents, families, and multidisciplinary teams to develop and implement individualized care plans, ensuring that each resident's emotional, social, and psychological needs are met.
Key Responsibilities:
  • Conduct comprehensive assessments to identify residents' needs and preferences.
  • Develop and implement individualized service plans in collaboration with residents, families, and healthcare providers.
  • Provide emotional support and counseling to residents and their families.
  • Facilitate support groups and educational workshops to promote mental health and well-being.
  • Advocate for residents' rights and ensure they have access to necessary resources and services.
  • Collaborate with nursing staff, activity coordinators, and other team members to ensure holistic care.
  • Document and maintain accurate records of resident interactions, care plans, and progress.
  • Stay informed about community resources and services that can enhance resident care.
Qualifications:
  • Bachelor's degree in Social Work (BSW) required; Master's degree (MSW) preferred.
  • 2-3 years of experience in a social work role, preferably in a senior living or healthcare setting.
  • Valid state social work license (LCSW or equivalent) preferred.
  • Strong communication and interpersonal skills, with a compassionate approach to care.
  • Ability to work collaboratively within a multidisciplinary team.
  • Knowledge of geriatric care issues, community resources, and healthcare regulations.
  • Proficiency in documentation and computer skills.
